Add Form

Step 1 - From admin Dashboard, navigate to Donations--> Add Form

Step 2 - Form donate will display : Please enter title and add categories, tags of donate

Step 3 - You can see Donation Form Options : Please enter all configs in box.

Step 4 - Click to button "Publish" when donate is ready
